Both the medical provider and the patient can do this.

The whole point of this assessment is to see how willing the patient is to participate in their care, as well as what sort of informal support they already have from family members or friends.
The nursing assessment is carried out by a licensed and registered nurse in New York and involves evaluating what the patient's long-term care needs are from a medical standpoint.
